Welcome to the Payment Plan Application.

If you do not have the funds for the start-up fees, do not complete this form. It may take a few days for processing to assess the full amount of fees on your case.

Per Florida Statute 28.24(26)(c), a one-time administrative fee of $25.00 and a 10% down payment will be charged per case to establish a payment plan that is due at the time of sign up .  If you have multiple cases and want to combine them into one monthly payment, please fill out an application for each case.  Restitution fees cannot be included in a payment plan, if you have been ordered restitution on your case, please contact us for more information. The monthly payment for a single case payment plan is between $25- $50 based on income. Multi case payment plans with two, three or four cases will have a $50 monthly payment. Any multi case payment plans with five or more cases will have a $100 monthly payment.

IF YOUR RECEIVED A TRAFFIC CITATION FOR FAIL TO PROVIDE PROOF OF  DRIVER'S LICENSE, INSURANCE OR REGISTRATION, PLEASE CONTACT OUR OFFICE BEFORE FILLING OUT A PAYMENT PLAN APPLICATION. FOR ANY TRAFFIC CITATIONS, YOU HEREBY WAIVE THE OPTION TO ELECT TRAFFIC SCHOOL OR ELECT A COURT HEARING.

IF YOU ARE ON A PRETRIAL INTERVENTION PROGRAM (PTI) DOES NO REQUIRE A PAYMENT PLAN. IF YOU SIGN UP FOR A PAYMENT PLAN WHILE ON A PTI PROGRAM STATUS, YOU ARE AGREEING TO THE PAYMENT PLAN OBLIGATIONS AND RULES.
